I encourage staff to report even small changes in a resident. I maintain an open and approachable leadership style to support that expectation. Instead of staying in my office, I remain visible and present in daily operations.

I regularly engage team members in their work areas and ask specific questions about residents. These conversations show that I value their insights. As a result, staff feel more comfortable sharing concerns. Over time, this visibility builds trust and strengthens accountability across the community.

Stand-up meetings and rounding play a critical role in early detection. During these touchpoints, team members share real-time updates about residents. We identify concerns early and address them together.

Whenever appropriate, we include residents and families in those discussions. Early conversations allow us to plan thoughtfully instead of reacting to emergencies. Families also appreciate prompt updates. When they hear about changes early, they share important history and context. Their perspective often helps us act faster and more effectively.

We may recommend adaptive equipment, adjust care, or involve therapy services. These timely actions reduce fall risk and prevent avoidable injuries.
